Olive Leaf Extract
Blood pressure support, antioxidant protection, and antimicrobial activity

Olive leaf extract comes from the leaves of Olea europaea — the same plant that produces olive oil — and has been used medicinally in Mediterranean cultures for millennia. While olive oil gets most of the press for cardiovascular health, the leaves contain a far higher concentration of bioactive polyphenols, particularly oleuropein. Standardized olive leaf extract delivers therapeutic levels of these compounds that would be impossible to obtain from diet alone.
Oleuropein is the primary active compound, and its main metabolite — hydroxytyrosol — is one of the most potent natural antioxidants identified to date. The cardiovascular mechanisms are multifactorial: oleuropein inhibits ACE (angiotensin-converting enzyme), relaxes smooth muscle in blood vessel walls through calcium channel modulation, reduces LDL oxidation, and has anti-platelet effects. Additionally, oleuropein and hydroxytyrosol disrupt bacterial and viral cell membranes, giving olive leaf extract a broad-spectrum antimicrobial profile that is well-documented in laboratory research, with emerging clinical evidence.
Blood Pressure Reduction
A head-to-head RCT compared standardized olive leaf extract to captopril (a first-line ACE inhibitor drug) in Stage 1 hypertension patients and found comparable blood pressure reductions over 8 weeks. Olive leaf extract works via ACE inhibition and calcium channel modulation, making it one of the better-evidenced botanical options for mild hypertension.
Antimicrobial Activity
Oleuropein and hydroxytyrosol have demonstrated broad-spectrum activity against bacteria (including Staphylococcus, E. coli, Helicobacter pylori), fungi (Candida), and certain viruses. The mechanism involves disruption of microbial cell membranes and interference with viral replication. Strong in vitro evidence; clinical human studies are ongoing.
Antioxidant Protection
Hydroxytyrosol, the primary metabolite of oleuropein, has one of the highest ORAC values of any natural antioxidant. It protects LDL from oxidation (a key step in atherosclerosis), reduces oxidative stress markers, and protects cell membranes from free radical damage. This likely underlies many of olive leaf’s cardiovascular benefits.
Blood Sugar & Lipid Support
Several clinical studies show olive leaf extract improves insulin sensitivity and reduces fasting glucose in prediabetic and type 2 diabetic populations. It may also modestly reduce LDL cholesterol and triglycerides, making it a useful complementary supplement for metabolic health when combined with diet and lifestyle changes.
- Always choose standardized extract (20%+ oleuropein) — unstandardized products are unreliable
- Take with food to minimize any gastric discomfort; oleuropein can be irritating on an empty stomach
- If you take blood pressure medication, monitor BP closely — additive effects are possible
- Allow 4–8 weeks to assess blood pressure effects; benefits build gradually
A 2011 RCT by Susalit et al. published in Phytomedicine enrolled 232 patients with Stage 1 hypertension in a double-blind, active-controlled trial comparing olive leaf extract (EFLA 943, 500mg twice daily) directly to captopril (12.5–25mg twice daily). After 8 weeks, both groups showed statistically significant and comparable reductions in systolic and diastolic blood pressure, with the olive leaf extract group showing a mean systolic reduction of 11.5 mmHg. This is one of the only botanical supplements to demonstrate equivalence to a pharmaceutical antihypertensive in a well-designed RCT.

Olive leaf extract and garlic are complementary — different mechanisms make combining them reasonable for cardiovascular support.
