MSM (methylsulfonylmethane) is a naturally occurring organosulfur compound found in small amounts in many foods — including fruits, vegetables, grains, and milk — but at concentrations far too low to produce therapeutic effects from diet alone. It is one of the most popular joint supplements in the United States and one of the few in that category with genuine clinical evidence behind it. MSM is odorless, stable, and well-tolerated even at high doses over extended periods.
MSM works through two primary mechanisms. First, it provides bioavailable sulfur — a critical building block for collagen (the structural protein in cartilage, tendons, and skin), keratin (hair and nails), and glutathione (the body’s primary antioxidant). Sulfur cross-links stabilize the three-dimensional structure of these proteins, and adequate sulfur availability supports their synthesis and maintenance. Second, MSM reduces inflammation by inhibiting NF-κB signaling and reducing the production of pro-inflammatory cytokines including IL-1β and TNF-α. This dual action — structural support plus anti-inflammatory activity — explains why it is particularly effective for osteoarthritis and post-exercise muscle soreness, where both pathways are relevant.
Osteoarthritis Pain & Function
Multiple RCTs demonstrate that MSM at 1.5–3g/day significantly reduces joint pain, swelling, and stiffness in osteoarthritis patients after 12 weeks. Improvements in physical function scores (WOMAC, VAS pain) are consistent across trials. The combination with glucosamine has shown additive benefits in some studies.
Exercise Recovery & Muscle Soreness
MSM reduces delayed onset muscle soreness (DOMS) and oxidative stress markers after intense exercise. Trials in runners and cyclists show lower pain ratings and faster recovery with MSM supplementation, likely through both antioxidant and anti-inflammatory mechanisms. An effective option for active individuals with joint stress.
Skin, Hair & Nails
Sulfur is essential for keratin and collagen synthesis. MSM supplementation has been associated in some studies with improved skin elasticity, reduction in fine lines, and stronger hair and nails. These benefits require longer use (3–6 months) and the evidence base is smaller than for joint pain, but the biological rationale is strong.
Antioxidant Support
MSM increases glutathione production — the body’s master antioxidant — by providing the sulfur needed for its synthesis. It also reduces markers of oxidative stress in clinical trials. This antioxidant dimension complements its direct anti-inflammatory activity and contributes to its broad tissue-protective effects.

A 2006 double-blind RCT by Kim et al. published in Osteoarthritis and Cartilage enrolled 50 men and women with knee osteoarthritis and assigned them to 3g MSM twice daily (6g total) or placebo for 12 weeks. The MSM group showed statistically significant improvements in pain (WOMAC pain subscale: 25.1% reduction vs. 12.9% placebo) and physical function (WOMAC function subscale: 23.7% vs. 8.1% placebo). No serious adverse events were observed in either group, reinforcing the compound’s safety profile.
