Iron

The most commonly deficient mineral in the world – critical for oxygen transport and energy, but dangerous when taken by the wrong people.

Overview

Iron is an essential mineral that your body uses primarily to make hemoglobin – the protein in red blood cells that carries oxygen from your lungs to every tissue. It's also required for myoglobin (the oxygen-storage protein in muscle), energy metabolism, and immune function.

Iron deficiency is the most common nutritional deficiency worldwide, affecting an estimated 2 billion people. It's especially prevalent in premenopausal women, pregnant women, vegetarians and vegans, and endurance athletes. Left unaddressed, it progresses to iron-deficiency anemia – characterized by fatigue, weakness, brain fog, pale skin, and reduced exercise capacity.

Iron comes in two dietary forms: heme iron (from animal sources – meat, poultry, fish), which is highly bioavailable at around 15–35% absorption, and non-heme iron (from plant sources and supplements), which is absorbed at a much lower rate of 2–20% and is significantly affected by other foods in the same meal.

Who Actually Needs to Supplement?
Test first. Always.

Unlike most supplements, iron is one where supplementing without confirmed deficiency can cause real harm. Iron overload accumulates in organs and is linked to cardiovascular disease and liver damage. A simple blood test (ferritin + hemoglobin) tells you definitively whether you need it.

This is not a "can't hurt" supplement for most adult men or postmenopausal women.

  • Premenopausal women (especially heavy periods)
  • Pregnant and breastfeeding women
  • Vegans and vegetarians
  • Endurance athletes (especially female runners)
  • Anyone diagnosed with iron-deficiency anemia
  • People with malabsorption conditions (celiac, IBD)
Probably don't need it:
  • Adult men with normal ferritin levels
  • Postmenopausal women (unless deficient)
  • Anyone with hemochromatosis (iron overload disorder)
  • People eating a varied diet with regular red meat consumption
Forms of Iron – Which to Choose

Not all iron supplements are created equal. The form determines how well it's absorbed and, critically, how hard it is on your stomach.

Ferrous Bisglycinate (Best Choice)
Chelated iron bound to glycine. Superior absorption, significantly fewer GI side effects (nausea, constipation) compared to ferrous sulfate. Higher cost but worth it for most people who have struggled with other forms.
Ferrous Sulfate (Most Common)
The standard pharmaceutical form – inexpensive and effective. Good absorption but commonly causes constipation, nausea, and dark stools. Often prescribed for anemia treatment. Take with food if stomach upset occurs.
Ferrous Gluconate
Slightly lower elemental iron content per tablet than ferrous sulfate but generally gentler on the stomach. A reasonable middle-ground option.
Ferric Iron (Avoid as first choice)
Ferric forms (iron oxide, ferric pyrophosphate) are less well absorbed than ferrous forms. The body must convert ferric to ferrous iron before absorption – an inefficient step. Often used in food fortification rather than supplements.

Therapeutic dosing for confirmed deficiency (prescribed by a doctor) is typically 150–200 mg elemental iron per day in divided doses – significantly higher than the RDA. This should always be guided by blood tests and a healthcare provider.

The tolerable upper intake level is 45 mg/day of elemental iron for adults. Doses above this increase the risk of GI distress and, over time, organ damage from iron accumulation.

Maximizing Absorption

Iron absorption is highly variable and significantly influenced by what else you eat at the same meal. This matters more than which form you choose.

Increases absorption:
  • Vitamin C – the most powerful enhancer. 100–200mg taken together can double non-heme iron absorption
  • Acidic environment (empty stomach, citrus juice)
  • Meat, poultry, fish (even for non-heme iron in the same meal)
Decreases absorption:
  • Calcium – separate iron from calcium supplements or dairy by 2 hours
  • Tea and coffee – tannins chelate iron; wait 1 hour before or after
  • Phytates in whole grains and legumes
  • Antacids and proton pump inhibitors
Safety & Interactions

Iron is one of the few common supplements where the risks are real and well-documented at doses only moderately above the RDA. This isn't scaremongering – it's simply a nutrient the body stores without a good mechanism to excrete the excess.

Iron overload (hemosiderosis)

Chronic excess iron accumulates in the liver, heart, and pancreas. Hereditary hemochromatosis affects roughly 1 in 200 people of northern European descent – these individuals should never supplement iron without medical supervision. Even in the general population, long-term supplementation without deficiency is not benign.

Drug Interactions

Iron significantly reduces the absorption of many medications. Take iron at least 2 hours apart from: levothyroxine (thyroid medication), fluoroquinolone antibiotics (ciprofloxacin), tetracycline antibiotics, levodopa (Parkinson's), and bisphosphonates (osteoporosis drugs). This is clinically significant – tell your pharmacist you take iron.

Children & Accidental Overdose

Iron poisoning is a leading cause of fatal poisoning in children under 6. Keep all iron supplements stored securely and out of reach. Child-resistant caps are important but not foolproof.

Smart Pairings
Vitamin C – Essential Pairing
Take 100–200mg of Vitamin C alongside your iron supplement. It converts ferric iron to the more bioavailable ferrous form and can double absorption of plant-based iron. Many iron supplements already include Vitamin C – check the label.
Magnesium & Calcium – Take Separately
Don't take iron at the same time as magnesium or calcium supplements – they compete for absorption. Separate by at least 2 hours. This is one of the most commonly missed interactions in multi-supplement protocols.
Folate & B12
Iron-deficiency anemia and B12/folate-deficiency anemia often coexist – particularly in vegans and vegetarians. If you're treating iron deficiency, it's worth having B12 and folate levels checked at the same time.
