Garlic (Allium sativum) has been used medicinally for over 5,000 years across Egyptian, Greek, Roman, Chinese, and Ayurvedic traditions. It is one of the most extensively researched botanical supplements in modern medicine, with hundreds of clinical trials examining its effects on cardiovascular health, immune function, antimicrobial activity, and cancer risk reduction.
The key challenge with garlic supplementation is preserving bioactive compounds through processing. Raw garlic contains alliin, which converts to allicin when the clove is crushed or chopped — but allicin is highly unstable and degrades rapidly. Most of the cardiovascular and immune benefits in clinical studies come from aged garlic extract (AGE), which contains stable organosulfur compounds including S-allylcysteine that are well-absorbed and odorless.
Formed when alliin meets alliinase enzyme upon crushing. Potent antimicrobial and antioxidant. However, allicin is unstable — it degrades within hours and is largely destroyed by stomach acid. Most supplement tablets deliver little active allicin unless enteric-coated.
Formed during the aging process. Highly stable, well-absorbed, and odorless. S-allylcysteine is the primary compound responsible for the cardiovascular benefits seen in aged garlic extract clinical trials and has excellent bioavailability compared to allicin.
Multiple meta-analyses consistently show garlic supplementation reduces systolic blood pressure by approximately 5–8 mmHg and diastolic by 2–4 mmHg in people with hypertension. Effects are most pronounced in those with elevated baseline BP. A 2016 meta-analysis by Ried et al., covering 20 trials with 970 participants, confirmed these findings.
Garlic produces modest but consistent reductions in total cholesterol (approximately 8–10%) and LDL cholesterol. It also inhibits cholesterol synthesis in the liver and reduces platelet aggregation. Effects are more pronounced with aged garlic extract than with raw garlic powder.
A 2014 Cochrane review found that garlic supplementation may reduce the duration and severity of cold infections. A 12-week RCT found that daily garlic supplementation reduced cold incidence by 63% compared to placebo. Allicin's antimicrobial properties are active against a broad range of pathogens.
Garlic's organosulfur compounds reduce markers of oxidative stress including malondialdehyde and increase glutathione levels. Aged garlic extract specifically has been shown to reduce inflammatory markers including C-reactive protein (CRP) and interleukin-6, relevant to cardiovascular risk.

Bottom line on forms: For cardiovascular benefits, aged garlic extract (AGE) is the most clinically validated form. Look for products standardized to S-allylcysteine (SAC) content. Kyolic is the most studied brand. For immune/antimicrobial purposes, enteric-coated allicin tablets or fresh garlic are more relevant.

A 2016 meta-analysis by Ried et al. pooling data from 20 randomized controlled trials (970 participants) found that aged garlic extract significantly reduced systolic blood pressure by an average of 5.1 mmHg and diastolic by 2.5 mmHg in people with hypertension. A separate Cochrane review confirmed garlic’s role in reducing the duration and incidence of the common cold. Across the cardiovascular literature, effects are most consistent for aged garlic extract (standardized to S-allylcysteine), and require 8–12 weeks of consistent use to manifest.
