Creatine is a naturally occurring compound found in small amounts in meat and fish. It plays a critical role in producing energy during high-intensity, short-duration activities like weightlifting and sprinting.
While your body produces about 1 gram per day, supplementing with creatine monohydrate significantly increases muscle creatine stores — often by 20–40%. This is why it remains one of the most popular and effective supplements for athletes and fitness enthusiasts.

Walk down any supplement aisle and you'll see a dozen creatine variants promising faster absorption, less bloating, or "next-generation" results. The research doesn't back most of them up — creatine monohydrate remains the only form with decades of consistent human data behind it.
Creatine Monohydrate — the one to buy
Used in nearly every major clinical trial, including the studies behind the dosage numbers on this page. Cheap, stable, and effective. "Creapure" on the label refers to a German-manufactured monohydrate independently tested for purity, but generic monohydrate from a reputable brand works just as well.
Creatine HCl (hydrochloride)
Marketed on the claim that its higher water solubility means a smaller dose and less bloating. A 2025 placebo-controlled trial in elite handball and softball players tested this directly — creatine HCl produced no greater strength or body-composition gains than monohydrate, even at a matched low dose, and the authors concluded claims of its superiority are "unfounded and misleading." It typically costs more per gram of actual creatine.
Buffered Creatine (Kre-Alkalyn)
Sold on the claim that a higher pH prevents stomach breakdown, meaning better retention and fewer side effects at a lower dose. A controlled 28-day trial with muscle biopsies found no difference in muscle creatine content, body composition, or strength gains compared to standard monohydrate, at either the manufacturer's recommended dose or a matched dose.
Creatine Ethyl Ester — skip this one
This one isn't just "no better" — it's measurably worse. A controlled trial found creatine ethyl ester breaks down into creatinine (a waste product your body simply excretes) before it can be used, resulting in lower muscle creatine levels and worse strength and body-composition outcomes than monohydrate, plus higher serum creatinine as a byproduct of that breakdown.

Creatine has an excellent safety record in healthy adults, but it isn't a fit for everyone. A 2023 narrative review in Nutrients concluded that people with pre-existing kidney disease and a very low glomerular filtration rate "should refrain from creatine use" — the review notes there's still very limited research on creatine's safety in people who already have kidney disease, so this is a real gap, not a settled question.
Also use caution if you're on medications that stress the kidneys, and talk to your doctor first if you're pregnant or breastfeeding — not because of documented harm, but because there simply isn't enough research in that population to say either way.
One practical note: creatine supplementation can raise your serum creatinine on a routine blood panel, which measures kidney function. This is expected — creatine breaks down into creatinine naturally — and doesn't mean your kidneys are struggling. Just mention you take creatine to your doctor before a routine metabolic panel so it isn't misread as a red flag.

What to look for
- "Creatine monohydrate" listed as the only active ingredient — not buried inside a proprietary blend
- A clear gram amount per serving (5g is standard), not vague "creatine complex" labeling
- Third-party testing, especially if you're a tested athlete — look for NSF Certified for Sport or Informed-Sport on the label
- Micronized monohydrate if you want it to mix more easily — same compound, just a finer particle size, not a different (or better) form
Red flags
- "Proprietary blend" or "creatine matrix" that doesn't disclose how much of each form is actually in the product
- Marketing language like "20x absorption" or "next-generation" with no cited study behind the claim
- A price per gram of actual creatine that's dramatically higher than plain monohydrate, for a form (HCl, buffered, ethyl ester) that hasn't outperformed it in trials
- No manufacturing or testing information disclosed anywhere on the label or website
