Ceramides are lipids that make up roughly half the fat content of the outermost layer of skin, where they act as the "mortar" holding skin cells together. That structure is what keeps water in and irritants out -- when ceramide levels drop, whether from aging, harsh skincare, or dry climates, the skin barrier weakens and transepidermal water loss (TEWL) rises, which shows up as dryness, roughness, and increased sensitivity.
Topical ceramide creams have been a dermatology staple for years, but oral ceramide supplements -- typically sourced from wheat, rice, milk, or fermentation byproducts like wine lees -- work on a different premise: that supplying the raw lipid materials systemically can support the skin's own barrier-building process from the inside. Unlike a lot of oral beauty-supplement claims, this one has been tested directly with objective measurements, not just self-reported "skin felt better" surveys.
Increased Skin Hydration
A systematic review and meta-analysis of 66 randomized controlled trials on oral skin-care supplements found ceramides produced a statistically significant increase in measured skin hydration compared to placebo.
Reduced Water Loss (TEWL)
The same meta-analysis found a significant decrease in transepidermal water loss -- an objective, instrument-measured marker of barrier function, not a subjective feeling of "hydration."
Effective from Multiple Source Types
A 12-week randomized, double-blind, placebo-controlled trial using ceramides and glucosylceramides derived from wine lees extract found significantly lower TEWL versus placebo -- one of several distinct source materials shown to work.
Consistently Well-Tolerated
Across the published 12-week randomized trials, oral ceramide supplements were reported as safe with no significant adverse events, in line with their long history as a normal dietary lipid.

A 2022 systematic review and meta-analysis pooled 66 randomized controlled trials on oral skin-care supplements and found that ceramides produced a statistically significant increase in skin hydration and a decrease in transepidermal water loss compared to placebo. That finding is backed by individual trials across different ceramide sources -- a 12-week trial using acetic-acid-bacteria-derived ceramide found significantly improved stratum corneum hydration, and a separate 12-week trial using wine-lees-derived ceramides and glucosylceramides found significantly lower TEWL versus placebo.
