Barley grass is the young, leafy shoot of the barley plant, harvested well before the grain forms and sold as a juice powder or supplement — a close cousin, both botanically and in the marketplace, to wheatgrass. It's marketed heavily around antioxidant content and general "green superfood" positioning, and it's worth separating out which parts of that reputation hold up to scrutiny and which are still thin.
The antioxidant claim is genuinely well-supported: barley grass juice contains meaningful concentrations of compounds like superoxide dismutase and has shown real antioxidant activity in laboratory testing. The cholesterol-lowering claim, often attributed to its beta-sitosterol content, is more preliminary — most of the strongest supporting data comes from animal studies rather than large, well-controlled human trials, which is an honest distinction worth making before assuming it works the way the marketing implies.

A 2018 review of barley grass's functional ingredients and their role in chronic disease found that antioxidants including superoxide dismutase are present in high concentrations in green barley juice and have demonstrated protective effects against oxidative stress in laboratory research. The same review notes preliminary animal and clinical data showing lowered total and LDL cholesterol, attributed in part to beta-sitosterol content, at doses around 15g/day of dried barley leaf extract — while being explicit that more rigorous, larger-scale clinical studies are still needed to confirm these effects.
