Vitamin K2

The missing partner to Vitamin D3 – directs calcium into bones and teeth where it belongs, and away from arteries where it causes harm.

Overview

Most people know Vitamin K from its role in blood clotting – that's primarily Vitamin K1 (phylloquinone), found in leafy greens. Vitamin K2 (menaquinone) is a different form with a substantially different function: it activates proteins that regulate where calcium goes in the body.

K2 activates two critical proteins: osteocalcin, which helps bind calcium to the bone matrix, and Matrix Gla Protein (MGP), which prevents calcium from depositing in soft tissue, including arteries. Without adequate K2, calcium absorbed through diet or supplementation can end up in the wrong places.

K2 is found naturally in fermented foods (especially natto, a Japanese fermented soybean dish with extremely high concentrations), some aged cheeses, and to a lesser extent in egg yolks and liver. Most Western diets provide very little. This is why supplementation has become relevant, particularly for anyone taking Vitamin D3.

The Vitamin D3 Connection
Why D3 users need to know about K2

Vitamin D3 significantly increases calcium absorption from the gut. This is largely why D3 supplementation improves bone density. But increased calcium absorption is only beneficial if the calcium ends up in bones – not in arteries and soft tissue.

K2 is what ensures that calcium routing. Without sufficient K2, high-dose D3 supplementation may contribute to arterial calcification over time. This remains an active research area, but the mechanistic case is compelling and K2 is well-tolerated with essentially no downside at normal doses.

If you're taking Vitamin D3 at doses of 2,000 IU or higher and you're not also supplementing K2, this is the most commonly missed pairing in basic supplement protocols.

Potential Benefits
Strongest Evidence:
  • Bone density: Studies show K2 improves bone mineral density and reduces fracture risk – particularly relevant for post-menopausal women. Japan has approved K2 as a treatment for osteoporosis.
  • Arterial calcification: The Rotterdam Study (4,807 participants) found that high dietary K2 intake was associated with reduced coronary artery calcification and a 57% lower risk of dying from cardiovascular disease.
  • Dental health: K2 activates the same proteins in teeth as in bone, and some research suggests a role in cavity prevention – though this area needs more direct study.
MK-4 vs. MK-7 – Which Form to Choose
MK-7 (Recommended)
  • • Longer half-life (~3 days vs. a few hours)
  • • Once-daily dosing is sufficient
  • • Derived from natto fermentation
  • • Effective at lower doses (90–200 mcg/day)
  • • Better for most supplementation goals
MK-4
  • • Shorter half-life – requires multiple daily doses
  • • Used in some clinical research at higher doses (45 mg/day)
  • • Synthetic form available
  • • Higher doses needed for bone effects
  • • More appropriate for targeted therapeutic use under medical supervision

For most people supplementing K2 alongside D3 for general health, MK-7 at 100–200 mcg per day is the practical choice. Look for products derived from natural natto fermentation with verified purity.

K2 is fat-soluble and should always be taken with a meal that contains some fat. Taking it with your D3 supplement at breakfast is the simplest approach.

Safety Profile

Vitamin K2 has an excellent safety profile at normal supplemental doses. Unlike Vitamin K1, K2 does not have a documented upper tolerable intake limit, and toxicity has not been reported at doses used in supplementation (up to a few hundred micrograms of MK-7 daily).

Important: Blood Thinner Interaction

If you take warfarin (Coumadin) or any anticoagulant medication, consult your doctor before taking Vitamin K2. Vitamin K in any form affects the blood-clotting mechanism that warfarin targets – dosage adjustments may be needed. This is the one significant contraindication to be aware of.

Vitamin D3 – Primary Pairing
D3 and K2 are the canonical pairing. D3 increases calcium absorption; K2 directs where it goes. Every person taking meaningful D3 doses should consider K2 alongside it.
Calcium
If you supplement calcium, K2 is especially important for routing that calcium appropriately. The K2 + Calcium + D3 triad is the cornerstone bone-health protocol.
Omega-3 (EPA/DHA)
Both support cardiovascular health through different mechanisms – a natural addition to a heart-health stack.
Frequently Asked Questions
Do I really need K2 if I'm already taking Vitamin D3?
It's worth considering, especially at D3 doses of 2,000 IU or higher. D3 increases calcium absorption, and K2 is what directs that calcium into bones rather than arteries and soft tissue. This is one of the most commonly missed pairings in basic supplement protocols.
Should I choose MK-7 or MK-4?
For most people, MK-7 is the practical choice. It has a much longer half-life (about 3 days vs. a few hours for MK-4), so once-daily dosing at 100–200mcg is sufficient. MK-4 requires multiple daily doses and higher amounts to achieve similar effects.
Can I take Vitamin K2 if I'm on blood thinners?
Talk to your doctor first. If you take warfarin or any anticoagulant medication, Vitamin K2 affects the same blood-clotting mechanism warfarin targets, and dosage adjustments may be needed. This is the main contraindication to be aware of.
