Overview

Pterostilbene is a naturally occurring compound found in blueberries and grapes, structurally almost identical to resveratrol — the difference is two methoxy groups replacing two of resveratrol's hydroxyl groups. That small structural change matters a lot practically: it makes pterostilbene meaningfully more stable and better absorbed orally, with animal studies showing roughly 80% bioavailability compared to about 20% for resveratrol, and a longer plasma half-life.

It's marketed heavily as "the better resveratrol" on that bioavailability advantage alone. What often gets left out is the one real human randomized controlled trial that tested it directly found a genuine tradeoff worth knowing about before you buy it: real blood pressure reduction, but an unwanted rise in LDL cholesterol at the standalone dose tested.

In the main human RCT, the higher pterostilbene dose (125mg twice daily) reduced systolic blood pressure by 7.8 mmHg and diastolic by 7.3 mmHg over 6-8 weeks compared to placebo -- a clinically meaningful reduction for a supplement.

Pterostilbene's two methoxy groups slow hepatic breakdown compared to resveratrol, giving it a longer half-life (roughly 105 minutes versus resveratrol's ~14 minutes in human plasma) and more consistent absorption.

Antioxidant Activity

Preclinical research shows pterostilbene matches or exceeds resveratrol's antioxidant and anti-inflammatory activity in cell and animal models, though this hasn't been directly confirmed with human biomarker trials at the same scale.

Modest Weight Effect

In the same trial, participants not on cholesterol medication showed a small but statistically significant reduction in weight-adjusted measures with pterostilbene -- a secondary finding, not the study's main outcome.

Research Snapshot

A 2014 randomized, double-blind, placebo-controlled trial in 80 adults with elevated cholesterol tested pterostilbene at 125mg twice daily, 50mg twice daily, 50mg twice daily plus grape extract, or placebo for 6-8 weeks. The high-dose pterostilbene-alone group saw significant reductions in both systolic (-7.8 mmHg) and diastolic (-7.3 mmHg) blood pressure -- but also a significant LDL cholesterol increase of 17.1 mg/dL, an effect not seen in the grape-extract combination arm. A companion safety analysis from the same trial found pterostilbene well-tolerated with no serious adverse events.
