The specific absorption percentages often advertised for these formats are largely extrapolations, not findings from rigorous human pharmacokinetic studies. They may be reasonable to try, but there isn't solid head-to-head human data proving they outperform standard capsules for the outcomes that actually matter.

Active cancer: this is a genuinely debated, theoretical concern rather than a settled risk. Cancer cells, like healthy cells, rely on NAD+ to fuel rapid growth, so there's a legitimate biological question about whether boosting NAD+ could support tumor growth in someone with active cancer. Evidence is mixed and mostly preclinical — decades of human research on other NAD+ precursors (niacin, nicotinamide) hasn't shown increased cancer risk — but out of caution, anyone with active cancer should talk to their oncology team before starting NMN, and consider waiting until remission if unsure.
Pregnancy and breastfeeding: there's no meaningful human safety data in this population, so it's best avoided.
Human trials to date have mostly run 8-12 weeks, so long-term safety data beyond that window is still limited — worth keeping in mind for anyone planning years of continuous use.
