Betaine HCl
Supplemental hydrochloric acid for low stomach acid and poor protein digestion
Betaine HCl is hydrochloric acid (the same acid naturally produced in the stomach) bound to betaine as a carrier molecule. It's sold as a supplement to support people with hypochlorhydria — low stomach acid — a condition more common than most people realize, especially in older adults, those taking acid-suppressing medications long-term, and people with chronic stress or autoimmune conditions.
Stomach acid serves several critical functions beyond just breaking down food: it activates pepsin (the enzyme that digests protein), triggers the release of digestive enzymes from the pancreas, kills pathogens in food, and enables the absorption of key nutrients including iron, B12, calcium, magnesium, and zinc. When stomach acid is insufficient, all of these processes are compromised. Betaine HCl works by temporarily lowering stomach pH to support these functions — but it must be used carefully and is not appropriate for everyone.
Important: Betaine HCl is not the same as betaine (trimethylglycine/TMG), which is used for methylation and cardiovascular support. These are entirely different supplements.
Protein Digestion
Adequate stomach acid is required to activate pepsinogen into pepsin, the primary enzyme that breaks down protein. Low HCl means poor protein digestion — symptoms include bloating after high-protein meals, gas, and a sense of heaviness. Betaine HCl restores the acid environment needed for proper pepsin activity.
Nutrient Absorption
Many minerals and vitamins require an acidic stomach environment for proper absorption: iron requires HCl for conversion from Fe³⁺ to the absorbable Fe²⁺ form, B12 requires HCl to separate from food proteins, and calcium, magnesium, and zinc all absorb better at lower pH. Low stomach acid is a major underrecognized driver of mineral deficiencies.
Gut Barrier & Pathogen Defense
The stomach's acidic environment (pH 1.5–3.5) acts as a critical first-line barrier against pathogens in food and water. Low stomach acid increases vulnerability to H. pylori colonization, food poisoning, and small intestinal bacterial overgrowth (SIBO) — conditions that worsen digestive health further.
Paradoxical Reflux Relief
Counterintuitively, some cases of acid reflux and GERD are driven by low stomach acid, not excess. Insufficient acid delays gastric emptying and weakens the lower esophageal sphincter signal to close — allowing whatever acid is present to reflux. In these cases, restoring adequate acid can actually reduce reflux symptoms.
- Always take with a protein-containing meal — taking on an empty stomach can cause burning or irritation
- Do not take with NSAIDs, corticosteroids, or if you have ulcers, gastritis, or esophageal damage
- Many products include pepsin — this combination is more effective than HCl alone
- As digestion improves, you may need less over time — the goal is to support function, not replace it permanently
Hypochlorhydria (low stomach acid) is estimated to affect up to 30% of adults over 60 and is significantly more prevalent in people taking proton pump inhibitors long-term. A foundational study by Recker (1985) in the New England Journal of Medicine demonstrated that calcium absorption was significantly impaired in achlorhydric patients (those with no stomach acid) when calcium carbonate was used, but not when calcium citrate was used — illustrating directly how stomach acid governs mineral bioavailability. Clinical experience with Betaine HCl supplementation is well-established in integrative medicine, though large-scale RCTs remain limited.
