Bacillus coagulans is a spore-forming probiotic bacterium, and that spore is the whole story of why it's different from most probiotics on the shelf. Lactobacillus and Bifidobacterium strains -- the bacteria in most yogurt-style probiotics -- are relatively fragile outside the body, which is why so many probiotic products need refrigeration and still lose potency over time. That builds on a 2016 pilot trial in diarrhea-predominant IBS patients using strain MTCC 5856, which found significant symptom improvement over 90 days versus placebo, and a related 2018 pilot trial in IBS patients with major depression, which found the same strain improved both depression scores and IBS quality of life.
